Use a custom score combiner for the top-level clauses

If we the users enters multiple terms, we want to boost
resulst matching some of them above other results which
are scored highly only because of a single term.

Hence we apply a custom combiner which weighs in the
number of matching clauses for the top-level boolean
query.
